PhysioConditioning is a physiotherapy and strength training company limited by guarantee (no. 10653857). The registered address is 31 Mary Seacole Road, Plymouth, United Kingdom, PL1 3JY.
How do we collect information from you?
We obtain information about you when you contact us for information about our activity eg to make an appointment or book a place on a workshop or event. We may also collect information from you with your agreement when you take part in our community programmes or events. This will be collected as a part of evaluation purposes and as such we will have a contract with you outlining the nature of participation and evaluation processes.
We collect financial information of yours only if we are engaging you as a supplier. Any payments made by you for our workshops are processed either online through direct payments with your bank service or through a third party payment processor (eventbrite.co.uk). We do not harness any financial data through eventbrite.
What type of information is collected from you?
The personal information we collect might include your name, address, email address and age. The sensitive data we collect may include information about your health and lifestyle as related to your appointment.
How is your information used?
We may use your information to;

We review our retention periods for personal information on a regular basis. We are legally required to hold some types of information to fulfil our statutory obligations (for example for health or HR purposes). We will hold your personal information on our systems for as long as is necessary for the relevant activity, as long as is set out in any relevant contract you hold with us, or until you request to stop receiving information from us.
Who has access to your information?
We will never sell or rent your information to third parties. We will never share your information with third parties for marketing purposes.
Third Party Service Providers working on your behalf
We may pass your information with your agreement to third party providers for the purposes of your treatments and providing services to you on our behalf (eg other health services). However, when we use third party service providers, we disclose only the personal and sensitive information that is necessary to deliver the service and we have a contract in place that requires them to keep your information secure and not to use it for their own direct marketing purposes. Please be reassured that we will not release your information to third parties beyond PhysioConditioning core service network unless you have requested us to do so, or we are required to do so by law, for example, for safeguarding purposes, by a court order or for the purposes of prevention of fraud or other crime.
Your choices
You have a choice about whether or not you wish to receive information from us. We will only send you information if you have requested it or there is legitimate interest in our activity and services eg you have attended an appointment and we would like to follow up on your progress.

The accuracy of your information is important to us. We’re working on ways to make it easier for you to review and correct the information that we hold about you. You have the right to ask for a copy of the information PhysioConditioning holds about you (we will charge £10 for information requests to cover our costs in providing you with details of the information we hold about you).
Security
When you give us personal information, we take steps to ensure that it’s treated securely. Security precautions are in place to protect the loss, misuse or alteration of your information. We will take reasonable precautions to prevent the loss, misuse or alteration of information you give us. All paper records will be kept locked and digital files will be stored in files on a password protected computer with limited access. No cloud based files are used currently.
Communications in connection with our activities will usually be sent by e-mail. E-mail, unless encrypted, is not a fully secure means of communication. Whilst we endeavour to keep our systems and communications protected against viruses and other harmful effects we cannot bear responsibility for all communications being virus or risk free.
Non-sensitive details (your email address etc.) are transmitted normally over the Internet/email, and this can never be guaranteed to be 100% secure. As a result, while we strive to protect your personal and sensitive information, we cannot guarantee the security of any information you transmit to us, and you do so at your own risk. Once we receive your information, we make our best effort to ensure its security on our systems.

16 or Under
We are concerned to protect the privacy of children aged 16 or under. If you are aged 16 or under‚ we will always request that a parent or guardian attends initial appointments and please get your parent/guardian's permission beforehand whenever you provide us with personal information e.g contacting us for an appointment or booking onto an event. We will seek parental permissions for any consents, e.g evaluation or photo / media use.

Appointment Terms and Conditions

Please inform PhysioConditioning of cancellation due to illness or other as soon as possible - and within 24hrs of your session to cancel with no charge. Please do not attend if you are unwell. If you do not show for your session the fee will still be charged.
Please follow all cleanliness and other guidelines within the building and during any outdoor activity at all times.
It may be necessary for PhysioConditioning to cancel or reschedule sessions in response to unforeseen circumstances or illness.
Appointments will be arranged and invoiced for in advance. Payment is due either before or on the day of your appointment or within invoice timeframe if different.
Additional requested sessions will be scheduled as timetable allows.
No charge will be made for sessions cancelled by PhysioConditioning.
